Kent County Infant Health Initiative

The Kent County Infant Health Initiative (KCIHI) Inter-conception (IC) program is an enhanced case management program through the Kent County Health Department to ensure that high-risk women are in optimal health before becoming pregnant again.

What services are provided to women in the program?

  • Case management through in-home visits with a community nurse
  • Risk screening at enrollment and every three months
  • Education on topics that impact inter-conception health
  • Referrals for support services
  • Assistance in obtaining family planning services
  • Dental care
  • Healthy Women’s Resource Guide which contains educational information and resources to assist women before, during, and after pregnancy
  • Preconception Kits which contains free gifts
  • Gift cards for using family planning services

Who is eligible to participate in the program?

Women who live in Kent County and have experienced a poor pregnancy outcome in the past 12 months who aren’t currently pregnant are eligible. Poor pregnancy outcomes are defined as:

  • Premature delivery (before 37 weeks gestation)
  • Low birth-weight (under 2,500 gm or 5 lbs. 8 oz)
  • Infant death
  • Fetal demise or stillbirth after 20 weeks gestation (in the fourth month of pregnancy)
