Target Population

First time, low income, pregnant women of diverse cultures residing in Kent County.

Public Health Nurses provide intensive, home-based services to women who are pregnant and planning to have their first baby. Home-visiting services continue until the child's second birthday. The goals of the program are to improve pregnancy outcomes, improve child health and development and support family self-sufficiency by encouraging goal-setting, family planning, continuing education and securing employment. Referrals accepted from all agencies that work with pregnant women, including health care providers and human service agencies. Pregnant women may also call us directly to enroll in the program. Clients must enroll before the 28th week of pregnancy.

Nurse-Family Partnership is an evidence-based community health program. A study released in August 2015 found that families enrolled in NFP between 1996-2013 saw multiple benefits including fewer preterm births, a decrease in intimate partner violence, a reduction in number of abortions, an increased rate of immunization and a Medicaid cost savings of 8.5% per child from birth to age 18.
