Partially Breastfed Infants & Formula Fed Infants

Michigan WIC currently has an infant formula contract with Similac (Abbott). This contract is effective November 1, 2021 through October 31, 2026.

Please know that the amount of formula an infant may receive each month may vary from family to family.

The amount of cans of formula your infant will receive each month depends upon the brand/type of formula you have chosen to use, the age of your infant, and your breastfeeding status.

If you wish to switch to a different type of formula you will need to contact your WIC office in order for your monthly shopping list to be updated.

If another type of formula is neededor if your family has other special dietary needs your healthcare provider/Doctor’s office will need to complete a medical documentation form called the Michigan WIC Special Formula/Food Request Form (DCH-1325 Rev 9/2016).

You will need to FAX and/or deliver in person to your WIC office your completed Michigan WIC Special Formula/Food Request Form (DCH-1325 Rev 9/2016) in order for your monthly WIC benefits/groceries to be updated.
